Medical Plan (Health care, prescription drugs, dental care)

BFL CANADA is proud to join the “Fédération québécoise des massothérapeutes’’ (FQM) in order to provide insurance coverage to its members.

The provincial medical program does not cover several expenses such as dental care, vision care, paramedical services and much more. These expenses add up quickly and can leave you financially vulnerable.

The Health and Dental Insurance Plan offered to FQM members enables you to choose the type and level of coverage which are appropriate to you. This way, you only pay for the benefits which you need and choose.

The Health and Dental Plan for members of the FQM is underwritten by Manulife Financial.

Some coverage does not require any medical proof.

The Association Health and Dental Plan offer you coverage for:

  • Prescription drugs (portion not paid by the RAMQ)
  • Paramedical services
  • Dental care
  • Homecare and nursing
  • Ambulance services
  • Vision care
  • Accidental dental treatment
  • Medical equipment and supplies
  • Hearing aids
  • Services with the survivors
  • And more

Additional coverage

Coverage needs vary from one family to another. Therefore, we propose that you integrate benefits such as critical illness and disability insurance into your total financial planning strategy, in order to enjoy greater peace of mind.

Lifecheque Basic

A non-taxable cash benefit is paid if you are diagnosed with one of the conditions covered by the critical illness coverage during the policy period. There is a limitation for existing health condition.

Assured capital: $25,000, $50,000, $75,000

What triggers payment of benefits?

Diagnosis of any one of the following covered conditions, subject to survival of waiting period:

  • Cancer (Life-Threatening)
  • Heart attack
  • Stroke (Cerebrovascular Accident)
  • Coronary Artery Bypass Surgery
  • Aortic Surgery

When does payment begin?
After the policy-holder survives the 30-day waiting period after diagnosis

How are benefit paid?
One-time cash benefit

Assistance Services Benefit

Return of Premium Option (age restrictions apply)
Return of Premium option provides a refund of 100% of eligible premiums at expiry.


Personal Accident Disability Insurance

Offers flexible coverage that provides benefits if you are injured as a result of an accident. Sickness disability coverage also available.

Issue Ages: From 5 to 80 years, age on last birthday

What triggers payment of benefits: Total disability as a result of an injury caused by accident. Injury means bodily harm resulting directly from an Accident. Accident means a sudden, unexpected, unforeseeable, unavoidable external event.

Period of compensation for 2 years: If you were employed prior to the accident, Totally Disabled means that you are unable to perform your own occupation and you are not working elsewhere. Otherwise, it means you are unable to perform most of your routine daily activities.

Optional endorsement 3 years Extension or up to 65 years: Under this cover, Total Disability or Totally Disabled means you are unable to perform any occupation for which you are reasonably suited based on your education, training and experience, if you were employed prior to disability; or you are unable to perform all of your routine daily activities, if you were note employed.

Waiting period: As early as 1st day or after 30 or 120 days from onset of disability

How are benefits paid?

Monthly benefits payable during the selected benefit period while disability lasts.

  • 2 years (24-hour Compensation Plan – base plan)
  • Can be extended for 3 more years or to age 65

Benefit maximum

The monthly benefit corresponds to 75% of the insurable income, to amount of $6,000 per month.

Standard benefits

  • Partial disability benefits (maximum 6 months)
  • Recurrent Disability (considered recurring if within 6 months of last disability)
  • Vocational or Physical Rehabilitation (3 months maximum)
  • Non-occupational disability
  • Accident hospitalization benefit of $100 per day (1 year maximum)
  • $100 ambulance benefit (reimbursement of cost,1 per accident)
  • $10,000 accidental death benefit which increases $1,000 per year in force up to $20,000.
  • Waiver of premiums after 30 days.

Optional coverage (age restrictions may apply)

  • Sickness Disability for 2 or 5 years or to age 65
  • Accident Excess Medical
  • Accidental Death and Dismemberment
  • 20 year Return of Premium
  • Return of Premium on Death

Total disability insurance (injury or sickness)

Provides cost-effective protection against total disability, with optional partial or residual benefits. Built-in discounts for business owners.

Effective date of the insurance: The loss of revenue insurance because of sickness will take effect when it is approved by the insurer and the premiums have been paid.

Monthly amount: For insured aged 18 to 55 years: maximum $5,000
For insured aged 56 to 60 years: maximum $3,500

Benefits period: The benefit is payable for the duration of the annuity chosen, as long as disability continues

Waiting period: 30, 60, 90, 120 or 730 days. However permanent part-time workers as well as new owners cannot select 30 or 60-day waiting period.

Total disability: During regular occupation period, inability to perform the substantial duties of the regular occupation and not working elsewhere. After regular occupation period, inability to work in any reasonable occupation.

Period of usual employment: Usually 5 years (2 years with 2-year annuity coverage).

Standard benefits

  • Waiver of premiums after 90 days or elimination period, if less
  • Recurrent disability (6 months)
  • Vocational rehabilitation
  • Non-occupational disability
  • Accumulation of days for elimination period
  • Automatic coverage enhancements of 3% annually (issue age 18-50, standard risks only)
  • Assumed total disability with 0-day elimination period

Optional coverage (age restrictions may apply)

  • Premium Refund Rider (PRR)
  • Additional Insurance Rider (AIR)
  • Cost of Living Adjustments (COLA) 5%
  • Health Protection Rider (HPR) – Standard or Enhanced covering HIV, Hepatitis B


Business Overhead Expense Insurance

  • Guarantee the refund of covered overhead expenses of the company in the event of disability of a key employee.
  • Benefits payable after 30 days.


Out of Province/Canada Medical Emergency Travel Insurance

Refund of reasonable and usual medical expenses a result of emergency medical attention during your trip, to amount of $5,000,000.

  • The covered expenditures include hospital expenses, doctors’ fees, diagnostic fees, paramedical services, ambulance services, emergency transport by plane, transport to the patient’s bedside, and more.
  • Emergency service, 24/7 assistance wherever you are.
  • Cover for an unlimited number of trips, during the first 30 consecutive days of each trip.

Some exclusions, limitations, and definitions apply.

This document provides a summary of benefits offered along with some definitions, exclusions and limitations. In the event of discrepancy between this document and the wording of the policy, the latter will have precedence.
